If you have your NHS card it’ll say on there who it is.
Failing that, logging in to the NHS website or app should also tell you.

Can anyone post a PDF of the IBN please?
6FTS are a little…slow… in sending that one to us.
Ours just give surgery, no named GP
Now we need a way of uploading this to SMS so there is a ‘has valid Av Med’ box that can be seen by event organisers. Include option to scan and upload.
I suggested this through the CoC about 4-5 years ago, but the ‘too difficult’ answer came rolling back down the hill.
Could it not just be a qualification?
As long as AEFs don’t start expecting CCF to do that…
AEF staff need to see the completed AvMed1 forms for each cadet.
Evidence is also required that a cadet is enrolled, over 13 years and has parental consent to fly.
The SMS activity report shows enrolment and consent, but beware it also shows if a cadet has a declared medical condition. This may be a condition that is not required to be declared on the AvMed1 form….
The most common errors on presented AvMed1 forms are:-
Don’t forget presenting out of date forms. Over the last 2 days I’ve had Av Meds that are 3 versions out of date. As AEFs we should not fly cadets with out of date forms but they still keep comming.
I suspect it’s either historic forms on Wing Bader accounts not being updated or historic forms being taken out of a drawer.
Please stop using the wrong versions…it makes our life much easier.
Heads up - Av Med Form 1 (v4.0) is now out, the difference is stated as listing a header:
“Official Sensitive Personal When Complete”
When was the even removed? It always used to have the OSP heading and footing.
So, does this mean it now has to be printed blank and filled in with a pen, then kept in an envelope marked with the classification inside an unmarked envelope, or completed only on systems accredited by the MOD to that classification (i.e. not a cadet or parent’s personal device)?
We’re allowed to process OSP info over email…
But if printed, should certainly be kept in the safe!
Hmmm, if no “yes” box has been ticked, then surely no medical / sensitive information has been divulged - the cadet is “healthy” in all respects?
It’ll still have their name and other details on it, thus it is personal data still.
it is annoying that this has created a Ver4.0 rather than simply 3.1 given the scale of the change
It’ll give certain people a massive hard-on at the thought of being able to turn cadets away from gliding/flying because they don’t have the latest version!
Depends on the versioning setup they’re using. Quite often minor versions are used for drafts, and major versions for issued documents. Makes no odds to us really, 3.1 would have the same implications as 4.0.
absolutely it makes no difference, but when asked “whats the difference?” it is something which really makes very little impact (if any) on the chance of a Cadet going flying yet has caused a new version to be created
