So im aware of the 3 ways to pass their various music blue badges but wanted to ask is their an actual form that is required to be filled in/signed by their music teacher etc showing pass at level 3?
Would sight of the Grading certificate not be all that is needed?

This, a copy of the grading cert should be fine. You could even attach a scanned copy on their SMS profile when you add the qual.

yes thats what i was thinking just wondered if their was a specific form that had to be done.
If its a certificate of a grading - just provide the certificate. For Blue, if it is a music teacher/ instrument coach declaring you are playing to a Grade 3 Standard, then the best would be a letter with a school letterhead signed by a competent person (music teacher/ instrument coach) or even an email as long as the email is from a school account and has the signature block of the teacher’s competency.
