On attempt to request hearing protection through stores I was asked to present a ‘Proforma B’
Can any of you wizards out there, locate it? 
I’ve only found proforma A - for cadet uniform not B
I think it’s in ACP20a if you still have a copy (it’s been obsolete for over a decade) - similar to proforma A
The admin staff at WHQ should have a version they can email you.
